This one-page checklist helps individuals and caregivers record movements that may warrant evaluation by a health care professional. It describes signs associated with tremor, dystonia, akathisia, and tardive dyskinesia, including repeated mouth movements, finger movements, and rocking of the body. Yes-or-no boxes provide a simple way to document observations before an appointment. It explains that abnormal movements can have different causes and encourages professional assessment and routine monitoring for people taking antipsychotic medications. The checklist supports communication with providers but does not establish a diagnosis.
